Magee Rehabilitation Hospital opened in 1958 as the first free-standing rehabilitation hospital in Philadelphia. The Hospital has over 600 employees and is nationally recognized for outstanding programs in physical and cognitive rehabilitation, with comprehensive services for spinal cord injury, brain injury, stroke, orthopedic replacement, amputation, pain management and work injury. Magee serves as the federally designated Regional Spinal Cord Injury Center of Delaware Valley. Only 14 such centers exist in the country. Magee is a founding member of The Christopher Reeve Foundation NeuroRecovery Network, which provides state of the art rehabilitation therapy.

The Magee Outpatient Nurse Clinician functions to provide both direct and indirect patient care following the standards of professional performance as outlined by Magee Rehabilitation, the State of Pennsylvania Nurse Practice Act, and the Professional Scope and Standards of Rehabilitation Nursing. The Outpatient Nurse Clinician is self-directed to provide care to diagnose and treat the patient’s response to actual or potential health problems in a fast-paced outpatient and call-center environment.

The Outpatient Nurse Clinician is an active member of the interdisciplinary rehabilitation team and works collaboratively with physicians, patients, care givers, other interdisciplinary team members, and outside agencies to assist the patient to their highest level of independence. The Outpatient Nurse Clinician has ultimate responsibility to assure that the care is individualized and appropriate to the patient’s needs and that the care is carried out comprehensively according to the plan.  The position requires the ability to analyze, synthesize, triage, and evaluate the patient’s status, concurrent care needs, and incoming calls to ensure that the highest level of safe and effective patient outcomes are achieved.

ESSENTIAL DUTIES AND RESPONSIBILITIES (includes but not limited to):

  • Utilizes specialized knowledge and advanced critical thinking and clinical skills necessary to provide care for people with physical disabilities and chronic illnesses during in-person clinic visits and triaging all incoming patient phone calls.
  • Provides patient care and directs ancillary staff care activities based on nursing and rehabilitation theories within the established policies, procedures and guidelines of the Nursing Department, the Outpatient Clinic and the Hospital.
  • Coordinates educational activities and uses appropriate resources to implement individualized teaching with patients and their families utilizing all methods available including in person visits, telephonic, or telehealth practices.
  • Demonstrates knowledge in human growth and development with respect to the specific chronological age, developmental age, or social maturity of the patient.
  • Provides direction and supervision of ancillary nursing personnel, demonstrates professional judgment, uses problem solving techniques and time-management principles and delegates appropriately.
  • Contributes to always providing a safe and therapeutic environment and supports activities that promote the patient’s functional goals and the prevention of complications.
  • Evaluates the care needed for goal attainment for individual patients and communicates and documents appropriately.
  • Actively participates in team meetings and conferences as needed and disseminates up-to-date inter-disciplinary communication regarding the patient’s status.
  • Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and other multi-disciplinary team members and to ensure the fulfillment of requirements for legal documentation and reimbursement.
  • Acts as a resource, clinical mentor, and role model for nursing staff and students and participates in activities such as hospital-wide committees, outpatient committees, nursing committees, patient committees, projects and programs, and professional organizations that promote the improvement of nursing care and the advancement of professional rehabilitation nursing practice.
  • Applies nursing research to clinical practice and participates in nursing research activities.

 

OUTPATIENT CLINIC - SPECIFIC DUTIES AND RESPONSIBILITIES (includes but not limited to):

  • Executes prescribed medical regimens and use advanced critical thinking and problem solving strategies to provide care that is supportive and restorative while acting as an educator and counselor with a special emphasis on prevention of deterioration and maximizing independence.
  • Supports all Magee Outpatient Programs including SCI Lifetime Follow Up Care, Brain Injury, Concussion, Stroke, General Rehabilitation, Amputee, Pain, Complex Wound Care, Workers Comp, Medical Home, Virtual Life, Telehealth and others as developed.
  • Acts as the patient’s advocate protecting the patients’ rights of self-determination when obtaining informed consent.
  • Assesses health status, illness conditions, response to illness, and health risks of outpatients and their care givers using advanced skills in history taking, physical examination as needed, and analysis of objective findings.
  • Diagnosis human responses to actual or potential health problems and continually seeks validation of the etiologies of the patient care problems.
  • Assists the physicians with all outpatient care activities including: smooth and efficient patient flow, communication to facilitate patient satisfaction, patient preparation, obtaining accurate patient data including biometric data, medication lists, and active problem lists, and provides patient/family/caregiver education regarding medications and treatments.
  • Summarizes input on the patient’s clinical status for the physician during or after treatment encounters and executes all physician orders.
  • Assists the physician in performing highly complex clinical procedures including Botox and baclofen pump administration, activities associated with the PDMP (prescription drug monitoring program) data base including: witnessing narcotic contracts, and UDS (urine drug screens), lab procurement, and IMPACT testing.
  • Employs highly skilled patient telephone triage skills by demonstrating sound clinical judgement, advanced critical thinking skills, the ability to synthesize patient issues for the physician, the ability prioritize the workload, and the ability to differentiate between responsibilities within the scope of practice.
  • Assists the physicians by providing a safe and adequate synthesis of all patient-related phone calls and requests and triages patient care needs and responses accordingly.
  • Assembles appropriate patient data and documents all findings, interventions, and results appropriately in the medical record.
  • Provides accurate and appropriate patient data from the medical record to justify diagnostic tests, equipment and supply needs, and medications. Also supplies information for all medication and insurance Prior Authorizations and Appeals.
  • Enters orders for prescriptions and outpatient therapies. Performs chart reviews and collates required data for the pre-certification of inpatient admissions as necessary.
  • Obtains physician signatures on all documents as legally required for orders for equipment, supplies, Prior Authorizations, insurance forms, Physical Capabilities forms, and Return to Work forms.  
  • Responds in a timely manner to all patient messages and prescription line requests to insure safe and effective patient care. Anticipates and resolves any actual or potential patient issues.
  • Provides immediate and ongoing follow up phone calls for all patient issues until resolution is achieved and as required for specific patient programs.
  • Evaluates all outcomes of patient care and treatment to ensure that all care needs and issues are met.
  • Interacts with the Outpatient Case Managers whenever appropriate to assist in resolving all patient issues and needs related to the patient’s well-being and quality of life.
  • Demonstrates flexibility in job demands. Performs job duties outside of primary responsibilities whenever requested or assigned, deals with problems appropriately and in a professional manner, cooperates with other personnel to achieve departmental objectives and maintains good interdepartmental relations as a team member.
  • Maintains open channels of communication with manager and colleagues and keeps manager informed and apprised of all pertinent issues.
  • Executes best practice initiatives to maintain and improve patient satisfaction.
  • Maintains clinical competency and continues professional growth by participating in training and continuing education programs necessary for credentialing or re-credentialing within the identified time frames.
  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, benchmarking state-of-the-art and evidenced-based practices, and participating in professional organizations.
  • Completes all mandatory educational programs such as Healthstream. Attends required meetings, inservices or programs. Participates in performance improvement programs.
  • Completes self-evaluation and defines goals at the end of the probation period and annually.
  • Participates in nursing scholarly endeavors such as research, publishing, and presenting papers.
  • Maintains the privacy and confidentiality of all patient information in accordance with all legal and HIPPA requirements.
  • Demonstrates knowledge of safety plans to protect the well-being of self, co-workers, patients and visitors. Safety plans include the Department of Nursing safety plans, infection control, fire regulations, disaster plans, emergency plans, active shooter plans, and all plans listed in the hospital safety program.

REQUIRED SKILLS AND QUALIFICATIONS:

EDUCATION/LICENSURE/EXPERIENCE

  • Graduate of a Bachelor of Science degree in nursing (BSN) required. MSN preferred.
  • Licensure as a professional registered nurse (RN) in Pennsylvania.
  • Minimum of 5-7 years of licensed professional rehabilitation nursing experience is required.
  • Certification in rehabilitation nursing - CRRN is preferred or must be obtained within one year of hire.
  • Certified in Cardio Pulmonary Resuscitation (Basic Cardiac Life Support) required.
  • Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations and the specialty of rehabilitation nursing.

  • Requires excellent verbal, written, telephonic communication, and triaging skills which involve interactions with patients, peers, professionals, and/or the general public. Must be able to communicate articulately and comprehend written and verbal communications in English. Must have a customer service approach to meeting the needs of both internal and external customers in terms of quality and timeliness of work completion.
  • Must be able to function in a dynamic, rehabilitation team work environment. Contacts include patients/families, caregivers, physicians, interdisciplinary team members, students, referring agencies and case managers

 PHYSICAL DEMANDS AND WORK ENVIRONMENT:

  • Operates in a clinical outpatient setting. Physical demands include prolonged standing, walking, bending, frequent lifting and pushing heavy weight with proper body mechanics of 50 lbs. or greater.
  • Routinely uses standard office equipment such as computers, phones, copier, and fax machine.
  • Exposure to cleaning solutions, equipment, regulated medical waste and isolation areas.
  • Flexibility of work schedule and ability to organize and prioritize work is essential.

EQUIPMENT USED:

  • Manual and power wheelchairs, overhead mechanical lifts, scales, electric vital sign systems, bedside glucose testing, portable suction, intercom system, computer, fax, cell phone system, and overhead paging system and other medical and office equipment.
  • Proficiency and accuracy with office technologies, specifically all Microsoft Office: Outlook, Word, Excel, and PowerPoint.
  • Rexpert, Kronos, and CPSI HRIS Systems

 

EXPOSURES:

 

  •  General hospital exposure. Must follow standard precautions as outlined in the Hospital Safety Manual, Infection Control procedures and the Hospital Safety plan.
